Refund Request Form


Please take a minute to complete this form to start the refund process.

All registrations paid online will be refunded to the credit card used at the time of payment.  Registrations paid in office via cheque/debit/credit card, a cheque will be issued and mailed, or an e-transfer will be sent to the email address provided.  If you received funding, a portion of your fees will be reimbursed to the charity.

 

REFUND POLICY

The following will be refunded as per the dates below. 

100% of annual fee less admin fee - Until September 1st

50% of annual fee less admin fee - First skate - October 31st

NO REFUNDS WILL BE PROCESSED ONCE THE HOCKEY SEASON STARTS UNLESS RELOCATING OR MEDICAL REASON, WHICH WILL ONLY BE REFUNDED IF APPROVED BY THE EXCUTIVE DIRECTORS. 

TRYOUT FEES and CAMP FEES ARE NON-REFUNDABLE.
